肇建功勞章 - Foundation [of the State] Merit Medal

The ribbon designed from the flag of this government, is of 37mm watered silk, of 8mm yellow/4.5mm blue/2.5mm white/7mm red/etc. The colors on the flag were used to represent major ethnic groups in Mengjiang: blue for the Mongols; red for the Japanese; yellow for the Han and white for the "Hui" (the name given to the Muslims at that time).​

It looks like cavalier was KIA on October 6, 1937 (his rank was 歩兵上等兵 - Infantry Superior Soldier, posthumous rank 歩兵伍長 - Infantry Corporal).
His document for the China Incident Medal (that will be established only in 1939) bears the same date as Manchurian Incident Medal - October 6, 1937 (the day of his death).
Document for the Mengjiang Foundation Merit Medal bears standard date - September 1st, 1939.
