Quite often the manufacturer (or sellers) try to hide caverns and othes imperfections of die casting under artificial patina.
Nice example of such "strategy".

Interestenly on the last medal fakers used original "paw" from japanese red cross sociede medal marked with /i.

2nd variation.

Much better "paw' (no wonder since this is original silver "paw" from early red cross medal) and medals are usually polished to hide imperfections of die casting.
Note the traces on the "paw".

The "paw" was "borrowed" from red cross medal similar to this one.

Note how the color of patina on the medal is different from the patina on the "paw" + again traces of deformation of the "paw" when they tried to fixed it to the medal.
